Summary:

Whilst on a training flight, the engine, a Lycoming O-235-L23C piston engine, suffered a failure to the base of the No 4 cylinder, and the engine stopped. A successful forced landing was carried out at a disused airfield without further damage. Subsequent examination of the engine indicated that the fracture around the base of the cylinder was associated with pitting corrosion, although damage to the fracture surfaces precluded a detailed assessment of the fracture. Significant wear was also found associated with the camshaft. In January 2001, the manufacturer issued a Service Instruction for this engine type, in which the replacement of cylinder assemblies with new items with improved corrosion resistance was recommended, following a significant number of such failures.
